Your First Visit

We would like to thank you for choosing Associates in Neurology and will strive to make your appointments here as seamless and organized as possible.  We realize your time is valuable and will do our best to be on time for your appointments. In order to help us, please arrive to your visit at least 15 minutes prior to your appointment to complete registration. We ask that you bring a photo ID, your insurance card(s), an updated list of medications you are taking and any previous medical records that you can provide. We are participating providers with many different insurance companies including Medicare, Anthem, Medical Mutual, United Healthcare, Cigna and Aetna. Please contact our office for a complete list or you may contact your insurance company to verify participation. Please click below if you would like to download our New Patient Registration Form and Financial Policy. We ask that you complete the registration form (plus any additional forms that are appropriate) and bring with you to your appointment.

Payment

We are participating providers with many different insurance companies including Medicare, Anthem, Medical Mutual, United Healthcare, Cigna and Aetna. Please contact our office for a complete list or you may contact your insurance company to verify participation.

Patients are required to know the benefits and eligibility requirements of their individual plans. We are happy to assist with verification but request that any specific questions regarding insurance be directed to our billing department prior to your appointment so that we can assist you. We ask that co-payments be paid at the time of service. Please contact our billing department with any questions you may have at 440-946-1200 option 5.


Appointment Cancellations

While we realize that unforeseen events may cause an appointment to need to be rescheduled, we do ask that notification is given. We require a 24 hour cancellation notice for any appointments with a physician or for testing scheduled in our office. A fee may be charged if proper notification is not received.


Prescriptions

When calling our office to renew prescriptions, please call at least 3-4 days before you are out of your medication. Often time there are refills at your pharmacy, please check with them prior to calling our office, the pharmacy can also request refills from us.
